Application Checklist
Forms that must be completed and returned:
- Intent to Register
- Enrollment Application
- Arts Survey (Please complete survey for appropriate grade)
- Certification Page
Documents you must return with the packet (COPIES ONLY!!!):
- Photocopy of child's birth certificate
Original birth certificate must be presented when students are accepted.
- Photocopy of child's social security card
- Photocopy of child's immunization records
- Photocopy of a document with your child's name and address
- Photocopy of child's current and past report cards
- Most recent Standardized Test Scores - students in grades 1-8
